g220v2 backing plate broken

Hi there

I've just bought a small spot pad backing plate, when using this with a spot pad I was working an area and the screw hot ripped out of the plate and broke.

What did I do wrong? There wasn't any instructions of what washers and screws to use with the g220 considering it was a meguirars product so I used a washer at the plate end and the washer off the larger plate for the tool end.

Any ideas?
